I hereby give permission for my child to participate in tryouts and related baseball activities conducted or sponsored by On Deck Baseball.
I understand that participation in baseball tryouts and related athletic activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, thrown or batted balls, use of baseball equipment and facilities, and other risks that may result in injury. I knowingly and voluntarily assume these risks on behalf of my child.
I certify that, to the best of my knowledge, my child is physically able to participate and has no known medical condition that would prevent or materially impair safe participation. In the event of an injury or medical emergency, I authorize representatives of On Deck Baseball to provide or obtain reasonable first aid, emergency medical treatment, ambulance services, or other medical care deemed necessary when I cannot be reached. I understand that I am responsible for any costs associated with such care.
In consideration for allowing my child to participate, I hereby release and hold harmless On Deck Baseball and its officers, directors, owners, members, coaches, employees, agents, volunteers, representatives, successors, and assigns from claims, liabilities, damages, losses, or causes of action arising from my child's participation in the tryout or related activities, including claims arising from the ordinary negligence of the released parties, to the fullest extent permitted by law. This release does not apply to conduct that cannot lawfully be released or waived.
By checking the box below and submitting this form, I acknowledge that I have read and understand this waiver and release, voluntarily agree to its terms, and represent that I am the parent or legal guardian of the participating minor and have the legal authority to provide this consent.
